Why do you need all of this information about my loved one?

We use the vital statistical information you provide to create a death certificate proof. We send this proof back to you to review and certify that all of the information is correct before we file the death certificate with the state. All of the information we ask for is used for the death certificate and will be on the certified copies of the death certificates. If any of the information is not known, we can have “Unknown” on the death certificate, but the word “Unknown” will appear on the certified copies of the death certificates.
